Primary Care and Community Services What we do Primary care is the first point of contact, day or night, for more than 90% of care in the NHS in l practice is a core element of primary care but is not the only way to access services. The Community Health Team provides a range of clinical interventions to house bound residents of Tower Hamlets. Our community health teams (CHTs) are made up of nurses, physiotherapists, occupational therapists and healthcare assistants. They deliver holistic nursing and therapy services to patients, along with both planned and unplanned care.
